Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address, you can still register. Immigration status or nationality don’t matter – reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved.

In partnership with Doctors of the World UK, we will ensure that our practice offers a welcoming space for everyone who seeks to use our services.
Please ensure you are in our catchment area before completing any registration forms.

If you are in the catchment area and wish to register at the practice please download the initial Registration form, as well as completing the New Patient Questionnaire.
PLEASE NOTE: all forms must be handed to a member of the team.
You do not need proof of ID to register with a GP, but it might help of you have one or more of the following:
If you’re homeless, you can give a temporary address, such as a friend’s address, a day centre or the GP surgery address.
If you are arriving from the Ukraine and wish to register at the practice please complete the registration forms as above and in addition you will need to complete a medical questionnaire please Click here

From April 2015 all patients, including children, must be allocated a named accountable GP, who will have overall responsibility for the care and support that a surgery provides to them.
For all patients at Broom Leys Surgery your named GP is Dr Scrivens. This does not mean that you will only see your named GP, your care will still be provided by the most appropriate member of the clinical team here at Broom Leys Surgery. Having a ‘named GP’ does not prevent or restrict you from seeing any other doctor or nurse in the practice as you currently do. Continue to book your appointments with the practice in the same way that you do now.
